Case Manager – Twin Cities

Job description

The Case Manager – Twin Cities will work alongside Together for Good’s professional care team to provide support for vulnerable children and families facing crisis situations. The Case Manager will facilitate participant intakes, assessment of needs, and facilitate hostings. They will manage a caseload of participants that will consist of some crisis care, as well as ongoing respite care for participating families. They will engage with TFG volunteers and collaborate with other agencies and community partners when needed to better support a participant family.

Primary Responsibilities

  • Facilitate participant intakes and determine if TFG can provide support.
  • Facilitate coordination of hostings and maintain an organized calendar of hosting logistics.
  • Provide support and professional oversight for Host Families during a hosting.
  • Coordinate, refer and/or collaborate with other agencies and providers when needed.
  • Maintain case notes and documentation in CRM database.
  • Obtain feedback from participant families and Host Families following each service.
  • Submit CPS reports as needed.

Minimum Education and Experience

  • Bachelor’s degree. Degrees in social work, psychology, sociology or human services
    related fields welcomed.
  • Skilled in the process of identifying and assessing a wide range of participant needs.
  • Knowledgeable about local community resources and how to use those resources for the benefit of participants.
  • Experience working in and partnering with, and/or the desire to work with diverse
  • Experience with foster care, adoption or family social services preferred.
  • Experience supporting individuals with mental health concerns and/or a history of addiction.

  • Able to articulate the Christian faith.
  • Active member of a local church.

  • Passionate about TFG’s Mission to serve vulnerable children and families in crisis.
  • Strong preference for culturally competent candidates with a history of serving diverse populations.
  • Highly organized and efficient with attention to detail.
  • Excellent communication skills, both written and verbal.
  • Strong advocacy skills.
  • Knowledge of community resources.
  • Aptitude to respond professionally and navigate crisis situations.

Travel and Work Schedule

  • Available for in-office work Monday – Friday from 9am – 5pm. Occasional availability needed for evenings and weekends.
  • Must be willing to travel throughout the Twin Cities metro.


  • This is a 32-40 hour/week hourly position.
  • Mileage reimbursed at the federal mileage rate.
  • Position includes PTO, as well as additional paid Holidays.
  • This position does not currently include medical benefits.
